1. Policy Statement

Lucky Heavy Diesel Maintenance Limited (NZBN 9429050791576) is committed to maintaining the highest practicable standard of health and safety for its employees, contractors, clients, and any third parties who may be affected by our work. This policy is maintained in compliance with the New Zealand Health and Safety at Work Act 2015.

2. Roles and Responsibilities

The Director of LHD Maintenance has overall accountability for health and safety. Every employee and contractor has a duty to: take reasonable care of their own health and safety, follow all safe work procedures, report hazards and incidents promptly, use personal protective equipment (PPE) as required, and refuse to perform unsafe work. Clients engaging LHD Maintenance for on-site work are required to provide a safe work environment, maintain accurate site induction records, and notify LHD Maintenance of any site-specific hazards prior to commencement.

3. Hazard Identification and Risk Management

LHD Maintenance applies a hierarchical approach to hazard management: elimination > substitution > isolation > engineering controls > administrative controls > PPE. A pre-work site assessment is conducted at every location. Known hazards include: working beneath or near heavy vehicles, hydraulic pressure release, hot surfaces and fluids, electrical systems, and roadside working environments.

4. Personal Protective Equipment

Minimum PPE requirements for all field operations: safety boots (steel-capped, AS/NZS 2210), high-visibility vest where on or near traffic, safety glasses when working with fluids or components, nitrile gloves when handling oils, solvents, or hydraulic fluids, and hearing protection when operating noisy equipment. Additional PPE requirements apply per task-specific risk assessment.

5. Incident Reporting and Investigation

All incidents, near-misses, and injuries must be reported immediately to the LHD Maintenance director. Notifiable events (as defined under the HSWA 2015) will be reported to WorkSafe New Zealand without delay. All incidents are investigated to identify root causes and prevent recurrence. Records are retained for a minimum of 5 years.

6. Training and Competence

LHD Maintenance employs qualified diesel mechanics with relevant trade certifications. All staff receive H&S induction training on commencement and ongoing toolbox training as procedures or regulations change. Appropriate licences and certifications are maintained current for all equipment operations.

7. Site Induction Readiness

LHD Maintenance personnel are ready for site induction at any client location. A copy of this policy, current trade certifications, public liability insurance documentation, and site-specific JSA (Job Safety Analysis) forms are available on request. 8. Review

This policy is reviewed annually or following any significant incident, change in legislation, or material change in our operations. Last reviewed: 2026.
